Glucophage Dosage
Patients with Type 2 Diabetes have no fixed dosage of Glucophage or Glucophage XR or any other pharmacologic agent. The dosage of Glucophage or Glucophage XR should be administered based on the effectiveness and tolerance keeping in mind not to exceed the maximum recommended daily doses. For adults, the maximum recommended daily dose of Glucophage is 2550 mg. For pediatric patients of 10-16 years old, the maximum daily dose is 200 mg. The maximum recommended daily dose of Glucophage XR in adults is 2000 mg.
Glucophage XR should be given once daily with an evening meal whereas Glucophage should be administered in divided doses with meals. Both of these medicines should be started at a low dose with gradual dose escalation. This reduces the gastrointestinal side effects and also permits the identification of minimum dose. For adults, significant responses are not seen at doses below 1500 mg per day but it is highly recommended that patients start with lower dose of Glucophage and gradually they increase the dosage. A Glucophage tablet is 500 mg twice a day or 850 mg once a day given with meals. The increments should be 500 mg weekly or 850 mg every 2 weeks, up to a total of 2000 mg per day, given in divided doses. For pediatrics, the usual starting dose of Glucophage is 500 mg twice a day, given with meals. Dosage should increase by 500 mg weekly up to a maximum of 2000 mg per day.
For Glucophage XR the dosage is 500 mg once daily with the evening meal. Dosage increases should be made in increments of 500 mg weekly, up to a maximum of 2000 mg once daily with evening meal. A trial of Glucophage XR 1000 mg twice daily should be considered if glycemic control is not achieved.
